Question: What insurance programs does CCNJ participate in?
Answer: CCNJ is a participant in almost all insurance plans. For our patients who do not have any insurance coverage, we have very affordable and convenient payment plans. Please call our office and ask to speak to the office manager for more details.

Question: How soon can I get an appointment?
Answer: We provide prompt care. We will make every effort to see patients as soon as possible, which is usually the next office day.

Question: How do I request for an appointment?
Answer: The quickest way is to call the office at 732-287-0096. You can download a Registration Form and fax it at 732-287-3515 in order to expidite things at the time of the appointment. 

Question: What information should I bring with me on my first appointment?
Answer: Always bring your insurance card, photo ID and list of your medications, if any. If you have prior medical records, you should also bring them.

Question: What are the diseases and conditions treated by Dr. Izzat Shah at CCNJ?
Answer: Dr. Izzat Shah is Board-Certified in Internal Medicine and Cardiovascular diseases. He specializes in heart and circulations problems, blood pressure problems, diabetes related circulation problems, preventive care, cholesterol problems, chest pain and a multitude of other medical problems. For any other medical or surgical problems that are not in his area of expertise, you will be referred to an appropriate physician/specialist within your geographical area.
